You may download a copy of the electronic volunteer/intern application below in two formats. This form is to be completed and submitted to the local facility of your choice. Please consult the Facility Directory for a statewide listing of all Department of Corrections prison facilities.

Please contact the referenced individual's office of the local facility of your choice in order to:

• arrange to mail in your application or
• schedule a time to meet with the appropriate departmental staff to submit your application in person.
